Teen Scene Registration Form

 Please note:

1. K – 7th grade registration packets
   Please print and submit to the Beth Shalom Office the following forms:  Student Registration form, Photography/Videography Permission form, Tuition Payment/Fee Schedule form and Direct Debit Authorization form( if applicable).   Please submit these forms with your payment to receive the early bird prices by June 1, 2021.
2. Teen Scene  - Registration Form – Please fill in and submit.  The form will come directly to the Beth Shalom Office.
